Heart Function

The Heart Function program is designed for people who are living with heart failure.


Program Features

The Heart Function program is up to 16 weeks long and includes:

  • A safe and effective exercise prescription
  • Education about topics related to exercise and heart health
  • Support for people who are at different stages of heart failure

The program is delivered by a team that includes a cardiac rehab supervisor (trained in kinesiology, physiotherapy, or nursing), physician, dietitian, social worker, and a psychologist.

The program is delivered in small groups. The group meets one time each week for a regularly scheduled class. Your group leader — a cardiac rehab supervisor (CRS) — will follow the education schedule that you receive with your program passport.​​​​
